Truman Strike Group during its recent Red Sea deployment were preventable and placed more than 100 sailors at risk.
The instances included a friendly fire incident in which the USS Gettysburg fired on U.S. jets, a nighttime collision with a freighter near Port Said and two separate F/A18 losses tied to equipment failures and poor communication.
Investigators cited fatigue, limited training and ongoing staffing shortfalls as key factors behind the problems.READ MORE
